The Academy of Extreme Environment Medicine

Thrive in the unknown

Rationale

Humans have long been drawn to the wild and remote corners of the Earth, venturing beyond the safety of structured society. Yet, the professionalisation of healthcare in austere environments has historically been led by Northern Hemisphere programs.

To bridge this gap, the Academy of Extreme Environment Medicine established the first Australasian-based Fellowship in extreme environment medicine. This pioneering program integrates both clinical and non-clinical competencies essential for providing effective healthcare in the world's most challenging settings. Graduates of the Fellowship emerge with the proven knowledge, experience, and leadership skills required to plan, support, and deliver care in extreme environments.

Definition

To establish clear governance and structure, it was essential to define what constitutes an extreme environment. To achieve this, we conducted an ad hoc Delphi study, drawing on a broad range of available references and expert input. Through this iterative process, we arrived at the following consensus definition:
"An extreme environment is characterised by an environment devoid of infrastructure such as a lack of hospitals, roads, transportation and food sources with extended field care and long evacuation required, and / or conditions beyond the optimal range for the survival of humans. For example, long distances, extreme cold and extreme heat, high radiation, low partial pressure of oxygen, high pressure or saturating salt concentrations".

A Quick History

The Academy of Extreme Environment Medicine was conceived in 2020 during the global lockdowns and publicly launched in 2022. Initially centred around the development of a core curriculum, the Academy has since evolved to establish rigorous standards for recognising qualifications and experience in extreme environment medicine. It accredits relevant conferences, courses, and training programs that align with its dynamic curriculum, encompassing both clinical and non-clinical competencies. Participants earn credits through approved activities, with Fellowship representing a distinguished professional qualification and formal recognition of practical expertise. It remains the only Fellowship of its kind in the Southern Hemisphere dedicated to this specialised field.
